About Bloodwork

Central Athlete is excited to offer a variety of different blood tests to members through our partners at Direct Labs. To order blood work online, follow the instructions below:
  • Click on this link
  • Register an account
  • Once registered, click Order Test
  • Add the tests needed to the shopping cart and proceed to checkout
  • Once the requisition is generated, a notification is sent and you can log into your account and print the requisition. You must have requisition before going to Quest Diagnostics. You can find a lab in your area by utilizing the Lab Locator. If a Specialty Kit is ordered, it will be mailed to you.
When the results are available you will receive an email letting you know the results have been uploaded to the same account, so you MUST keep your login handy to get into the account. Then, you can schedule a bloodwork analysis with Central Athlete Head Coach Jesse O'Brien here.
If you would like to release the results to someone besides yourself (like your coach), you can click on the HIPAA Release form and put in the recipient's information (email address and/or fax number).
If a HIPAA is submitted for you to receive results, they will be emailed, mailed, or faxed to you.
If you want to submit to your insurance companies for possible reimbursement, you can request a CPT-coded receipt when you place your order.  Please understand that the only Diagnosis code we use is Z00.00 which is general wellness since we do not treat or diagnose.  If your insurance plan doesn’t cover wellnesses, then you will not be reimbursed. The diagnostic code cannot be changed on our receipt. It's always a good rule of thumb to call your insurance company to see if they will reimburse for wellnesses with Diagnostic code Z00.00 before ordering.
